In Northwest Territories
Driver's licence and ID card
Changes to an NWT driver's licence or General Identification Card, including an address change, must be done in person at a driver and vehicle office. Bring your current licence and the supporting documents the page lists.
Health card
If your address changes, notify the Health Services Administration Office immediately so your coverage is not interrupted. Use the Notification of Change/Update Application Form, which is also available at hospitals, health clinics and health centres.
Health coverage when you move here
If you move permanently to the NWT from another province, you typically become eligible for the NWT Health Care Plan on the first day of the third month after the month you became a resident. Apply for an NWT Health Care Card.

Legal name change
The Registrar General of Vital Statistics at the Health Services Administration Office in Inuvik handles adult name changes using the Application for Change of Name. The page asks you to fax or email the completed form first as a draft for review, and lists a mailing address for sending the completed form.
